• We are no longer able to leave items for later pick up - please only register if you are able to be at the pick up event during the designated time.

• You must submit a separate form for each child that needs services.

• New placements must have placement paperwork uploaded within the form to receive services.

Please note: The form may still allow submissions after the deadline because the page remains active until the event concludes. However, registrations submitted after 5:00 PM Monday will not be accepted for this round of Mobile Connections.
